Pediatric tachycardia in a poorly responsive 10 kg, 9 month old male, diagnosis will ultimately be SVT, HR 240, narrow complex (QRS<0.09 sec), initial pressure 80/50, SpO2 90%. Child will be alert, grunting, RR 40.
Pt will be placed on nonrebreather 15L, SpO2 will then result 100%, IV/IO access will be attained, EKG will reveal SVT unresponsive to 0.1 mg/kg (1 mg adenosine), also with no response to 0.2 mg/kg (2 mg adenosine).
HR will increase to 260, pressure 55/30, child unresponsive, remains RR 40
Pt synchronized cardioverted at 0.5 J/kg (5J), no response, Pt synchronized cardioverted at 1 J/kg (10J), pt responsive, crying, BP 75/50, HR sinus 120, end of case
